Theoretical Basis of Etiology and Current Trends in Research for urolithiasis


Abstract
Dramatic advances have undoubtedly been made in the last 10 years in surgical procedures for removing urinary stones, but it has not much been accomplished in the field of prevention of further stone formation. Despite all the efforts spent on
research
into prevention of stone formation, the prevalance apparently continues to rise in many countries.
Some have been tempted to suggest that metabolic investigations of stone formers is hardly worthwhile when extracorporeal shock-wave lithotripsy(ESWL) is available. This is misconceived because only about one-third of stone formers need surgical
procedures, with the remainder passing small stones often with considerable pain and discomfort that they have no desire to repeat. Nevertheless, despite all the efforts spent on prevention of stone recurrences, success seems to remain small
compared to
the dramatic changes in surgical procedures. In reviewing theoretical basis of etiology and to try and consider why the efforts of many have yielded apparently only very modest results.
